BLO visits to give the prefilled Enumeration Form

The BLO brings the pre-printed Enumeration Form to the voter’s registered address as per the present voter record / EPIC address. This app helps your family keep sample filled forms ready at home before the BLO visit.

What the BLO brings A pre-printed Enumeration Form with existing voter details such as name, EPIC, address, serial number, part number, QR code and old photo.
What you prepare Current photo, DOB, family details, last SIR linkage details, and signature. Keep two recent passport-size photos for each family member ready.
Important home-readiness warning:
  • Do not travel far only for this unless it is really required.
  • Keep sample filled Enumeration Forms ready at home for all family members.
  • If the BLO visits when you are not there, any adult family member can submit/sign the Enumeration Form as allowed in Annexure III.
  • Keep the BLO-signed second copy or acknowledgment safely after submission.
Privacy promise: This page does not collect, upload, store, or submit personal information. Everything typed here stays inside this browser session.

BLO collects the filled form

This is the participation stage. The BLO collects the filled Enumeration Form. Documents are generally kept ready for notice stage, not forced into this preparation app.

Give one copy

Hand over the filled and signed Enumeration Form to the BLO.

Keep acknowledgment

Keep the second BLO-signed copy safely as proof that your family participated in the exercise.

Family member can help

The uploaded Enumeration Form allows the elector or any adult family member to sign with relationship mentioned.
Avoid common mistakes:
  • Mismatch in initials or spelling between voter record, Aadhaar, school certificate, passport and other documents.
  • Unclear photos or unclear document copies if later notice requires documents.
  • Missing required parent document for 1987–2004 or after 2004 birth windows.
  • Not keeping the BLO-signed acknowledgment copy.
Privacy promise: This button only checks readiness inside your browser. It does not submit anything to the government or to any server.
Step 4 of 6

Either accepted or notice is received

If previous SIR details are available and match, the form may move forward. If details are missing or mismatch, ERO/BLO may issue notice and then documents should be submitted.

    Step 5 of 6

Name in draft list comes or does not come

After the BLO/ERO processing stage, check whether the name appears correctly in the draft electoral roll.

    If name appears correctly

Keep the acknowledgment and still check the final list when published.

    If name is missing

Use the official claims/objections process immediately. Contact BLO/ERO and submit required proof copies as instructed.

    If details are wrong

File correction through official process, generally Form 8 where applicable. Avoid ignoring spelling and initials mismatch.
    Troubleshooting guide:
    • If recently deleted, ask officials about the appeal route with the District Election Officer.
    • If deletion is older or name is absent, ask whether Form 6 fresh registration is needed.
    • If details are wrong or mismatched, ask whether Form 8 correction is needed.
    • Keep self-attested copies and clear scans/photos of documents ready if official notice asks.
    Step 6 of 6

Name in final list comes or does not come

The final list is the last confirmation. Check it carefully even if the draft list looked correct.

If name is in final list

Your inclusion appears confirmed. Keep voter details and acknowledgment copy safely for future reference.

If name is not in final list

Do not delay. Contact BLO/ERO and ask for the official appeal, fresh registration or correction route based on your case.

    For future safety

Keep mobile linked to EPIC, maintain consistent name spelling, and preserve self/parent documents.
